Home Safety Checklist – in the last 12 months, have you:
- Fallen? Or nearly fallen?
- Had trouble getting into & out of chairs / bed / shower?
- Had trouble mobilising around your home?
- Had difficulty getting up or down stairs?
- Found it harder to recall or remember items, events or order of tasks?
- Found it difficult to complete all the household tasks each day?
- Planning to or recently undergone surgery? Been an inpatient at a hospital?
- Experienced limiting pain or fatigue for a significant period?
- Experienced a decrease in the distance you can walk?
- Experienced changes in your balance or vision?
OT Home Assessment
When we visit you in your home to complete a home assessment, our aim is to ensure you can be as independent and safe as possible in your own home. Our OT assessment will involve listening to you and understanding your challenges, goals and needs with completing your activities of daily living.
We will observe you in your home to assess and determine what type of assistance or supports you may need to complete your personal and domestic tasks. We will discuss practical solutions and recommendations and provide a report for you, your carer and family.
Assistive Technology and Equipment Prescription
Practical OT provides assessment, equipment trial and advice on specialised OT equipment, its safe use and purchase information to ensure you can be independent and safe when living in your home and accessing your community.
Home Modifications
Complementary to the OT Home Assessment, Practical OT provides advice and consultation with you, builders and designers, regarding minor and major home modifications to ensure you remain independent and safe inside and around your home.
Functional Capacity Assessment (FCA)
Practical OT provides FCA’s for children and adults accessing the NDIS to assess a person’s physical and cognitive ability to complete specific tasks. We complete the face-to-face assessment using a variety of tools to assess activities of daily living, fine and gross motor skills, ability to problem solve, follow instructions, engagement of tasks, routines, budgeting, and community access.
Practical OT will provide recommendations to assist with goal setting, planning for therapeutic inputs, and other supports.
